Step 1: Introduction
Please complete as much of this form as possible. Forms with more detailed responses will be easier to assess, and will allow the Scheduling Office to respond more promptly. After you submit your event request, you will receive an automatic e-mail from the Scheduling Office confirming that we have received the request. Please note that completing this form does not guarantee the Mayor’s participation or attendance at your event. The scheduling office will respond to all requests within 7-14 days. If the scheduling office e-mails you requesting more details for your event, please allow another 7 days for a response to your request. Please submit requests at least 2-3 weeks prior to the event’s occurrence. Due to executive order 3-11 (Prohibition of Acceptance of Gifts by City Officers and Employees), the Mayor is unable to accept any gifts at events. Please do not present the Mayor or his staff with any items of value.
